WCS Healthcare Partners is seeking a local contract nurse RN Case Management for a local contract nursing job in Bronx, New York.Job Description & Requirements

  • 21 hours/Week Flex Schedule
    
  • Evaluates the medical needs of individuals, devises interventions, assesses the outcomes of care, and conducts required treatments and specialized medical care.
    
  • Assists individuals with all aspects of daily living, including hygiene, skincare, and grooming.
    
  • Administers medications.
    
  • Conducts AMAP certification and recertification classes for IRA staff.
    
  • Supervises all AMAP-certified staff.
    
  • Executes the care plan specified in each individual's medical profile, including positioning, play, feeding methods, behavior adjustments, and attention to activities of daily living.
    
  • Delivers in-service training on medical topics as necessary.
    
  • Maintains monthly progress notes for each individual
    
  • On-Call from 7a-7p - responsible for giving guidance to DSP if any issues arise (calls will not be frequent)
    
    Job Requirements:
    
    • Updated First Aid/CPR certifications required
      
    • 1-2 years working with adults that has developmental disabilities preferred --
      
    • Must be ok with assuming responsibility for on-call (will not be called often) --
