  • Demonstrates knowledge of home care regulations and insurance reimbursement guideline.
  • Shows evidence of interdisciplinary communication.
  • Evaluates the plan of care and revises as needed.
    • Schedule Information
    • Maintains visit productivity standards.
    • Performs laptop transfer of data twice daily.
    • Transfers OASIS assessment within 24 hours.
    • Completes re-visit documentation and transfers back.
    • Follows scheduling process.
      • Additional Details
      • Knowledge of age-appropriate interventions.
      • Ability to work with individuals of diverse cultures and socioeconomic status.
