Job Description:
- Full time (35 hours) providing medication management to an adult only panel
- 15 minute med checks with 45-60 minute psychiatric evaluations, 18-20 patients per day
- Provide clinical care (medication management/psych evaluations) for patients to support efforts to achieve care plan goals
- Ensure appropriate transitions of care with external Article 31 providers, hospitals, etc
- Reviews case records to monitor quality of care
- Precept Psych NP students 1 day a week
- Standard business hours, M-F
- Community health setting, full age range of patients, diverse caseload
- Will consider going remote after completing a training/shadowing period of 2-3 weeks
